What should I bring to my appointment?
- Patients should bring all eye wear currently being used.
- Patients should bring a list of all prescription and over-the-counter medications currently used.
- Patients should bring all medical insurance cards and/or referrals acquired from your medical doctor.
- Payment of co-insurance, co-payments, deductibles, or non-covered services, when applicable, is required at the time of service.
Health Insurance and Vision Plans:
The Eye Institute accepts most Medicare and Medicaid plans, Blue Cross / Blue Shield plans, VSP, and many other vision and insurance programs. Verification of your plan benefits / coverage is required before your appointment. If required by your insurance or vision plan, please obtain the necessary referrals or authorizations from your primary care doctor prior to your appointment.
